Te aim of this study was to construct curcumin-loaded mesenchymal stem cell-derived exosomes (Cur-exos) and explore their efects on nonalcoholic fatty liver disease (NAFLD) models both in vitro and in vivo. Cur-exos were prepared using three common drug-loading techniques—incubation, sonication, and freeze–thaw cycles. Among these, Cur-exos prepared via repeated freeze–thawing demonstrated higher encapsulation efciency and drug-loading rates than those produced by other methods. In vitro, Cur-exos exhibited enhanced stability, were efciently taken up by hepatocytes, and mitigated palmitic acid (PA)–induced lipotoxicity in HepG2 cells. Moreover, Cur-exos signifcantly ameliorated liver damage in NAFLD model mice, reduced infammation, decreased reactive oxygen species (ROS) levels, and mitigated endoplasmic reticulum (ER) stress. Additionally, Curexos efectively regulated lipid metabolism disorders and improved impaired glucose tolerance. Overall, we demonstrate that, compared with free curcumin, Cur-exos signifcantly improve curcumin stability and ofer superior therapeutic efects in NAFLD mouse models. Tis study provides a novel approach for developing treatments for NAFLD.

本研究的目的是构建载有姜黄素的间充质干细胞衍生外泌体(Cur-exos),并探索其在体外和体内对非酒精性脂肪肝病(NAFLD)模型的影响。Cur-exos采用三种常见的药物载体制备技术——孵育、超声波处理和冻融循环。其中,通过反复冻融制备的Cur-exos表现出比其他方法更高的包封效率和载药率。在体外实验中,Cur-exos表现出更高的稳定性,被肝细胞有效摄取,并减轻了棕榈酸(PA)引起的HepG2细胞脂毒性。此外,Cur-exos显著改善了NAFLD模型小鼠的肝损伤,减少了炎症,降低了活性氧(ROS)水平,并减轻了内质网(ER)应激。此外,Cur-exos有效调节了脂质代谢紊乱,改善了受损的葡萄糖耐受性。总体而言,与游离姜黄素相比,Cur-exos显著提高了姜黄素的稳定性,并在NAFLD小鼠模型中提供了更优的治疗效果。本研究提供了一种新的方法。
